Hi Adisugu, the AI answer is correct. I haven't heard of to many people here in the US getting the PEM protocol prior to transplant. Most of us are offered the newer immunotherapy treatments before transplant. Many people are getting to MRD negativity without transplant. They are keeping transplant in reserve if needed later. I'm thinking they want to do a transplant, because you are so young. I had a transplant in 2017. I have been on Darzalex faspro and Pomalyst since before the transplant. Most patients in the US are offered a 4 drug induction including Darzalex, Velcade, Revlimid and dexamethasone.

What is the success rate of bone marrow transplant for a 48-year-old female patient with myeloma?

For autologous stem cell transplants in myeloma, the five-year survival rate has improved significantly to about 63 percent, up from 35 percent in earlier years. This improvement is largely due to better drug combinations used alongside the transplant.

At 48 years old, you would typically be considered a good candidate for an autologous stem cell transplant, as these are most commonly recommended for people under 65. However, your overall health and fitness level are the most important factors in determining candidacy.

The transplant can lead to:
- Longer remission periods
- Deeper response to treatment
- Improved progression-free survival (time before myeloma returns)

Keep in mind that while autologous transplants can provide extended remission for many people, myeloma does tend to return eventually. The goal is to achieve deep remission early, which often leads to longer-lasting remissions and more treatment options later.

Your doctor will evaluate your specific situation, including your organ function and overall health, to determine if this is the right approach for you.
