Hi Ted. If you click on your profile picture, it will take you back to your profile page. There’s a tab there called “Treatment”, which allows you can keep track of treatment journey. I haven’t used it much but it seems useful.
That's a really thoughtful idea, Ted! Staying organized with myeloma can make a big difference in managing care. While there isn't one universal template to point to, here are some practical approaches others have found helpful:

The American Cancer Society has many templates. One is called “Chemotherapy Side Effects Worksheet.” Daily Pain Diary, is another. You can find it at HTTPS://www.cancer.org/chemo
For blood tests, I print them out from MyChart and keep them in a binder. That way I can refer back to them, or share copies of them with other doctors not in the group.

You can connect your medical records from multiple sources directly there. They also have patient navigators, and a portal to find specialists and trials, etc.
They also have amazing videos and webinars. (They record them, so even if you can't watch them live, you'll get a link afterwards if you registers to attend.) And, they have "community meetings" (support group virtual meetings) that are specific to groups like MGUS and Smoldering; Light Chain MM, Newly Diagnosed MM, High Risk MM, Non-Secretory MM etc, etc. (These are not recorded.)
