In prep for a trial, I had a second bone marrow biopsy.
I'm curious how 2 biopsy results, in less than 3 months, indicates a 25% drop happening without any treatment.
I did discover. some online info that plasma cell distribution is inconsistent at best.

I have found with the right foods and supplements, I can move my numbers for the better. More pure water (I drink out of glass bottled water, not plastic) alkaline foods like cucumbers. If I’m not taking 1400 mg of calcium with Magnesium, D3 and K2, I’m in a mess of pain. I have read the same as you about different locations having different amounts in the biopsy, but it may be the lab person counting them too. Dr Brandy, who has MM, wrote a book called “Beat Back Cancer Naturally.” I highly recommend it.

Hi Ted. The location of the biopsy may matter. I had my first taken from my collar bone which was 10 to 13%. I had my second taken from my hip bone 2 years later which was 15 to 20%. I presumed that meant I was getting worse but my doctor said not necessarily the different locations could account for the different readings. Were both of your biopsies taken from the same location?

This sampling variability is a well-recognized limitation of bone marrow biopsies. The percentage of plasma cells found can differ depending on:

- Where exactly the needle was inserted
- How large the sample was
- The lab technique used to analyze it
- Natural fluctuation in plasma cell activity over time So a drop from 20% to 15% without treatment doesn't necessarily mean your myeloma improved — it may simply reflect that the second biopsy sampled a slightly different area with fewer plasma cells concentrated there.

This is actually one reason why doctors sometimes use additional tests alongside biopsies, such as MRI or PET-CT scans, to get a broader picture of what's happening throughout the bone marrow.

It's absolutely worth raising this question with your care team before the trial, as understanding your baseline accurately is really important.

Bone marrow results can vary from test to test. The % of myeloma cells can vary from location to location. The cancer cells are not evenly divided throughout the bone marrow.
