Dear All, I Know That The Question Is Really Difficult To Answer. A New Kidney Transplant With Myeloma With Kappa Chain In Low Levels?
My mum is with the following levels:
Kappa chain: 50.36 mg/L
Lambda chain: 22.59 mg/L
Kappa/Lambda rate: 2.23, it should be between 0.52 and 2.42
She is with daratumumab treatment (one dosis each one or two months, depending on doctor's decision.).
After a bone marrow biopsy, she got 0.0024% of clonal plasma cell cyIgL Kappa CD38+d (crowded) CD56* CD19- CD45- CD138+ Beta2micro+ CD117- CD81- This result is equal than the last bone marrow biopsy (0.0025% one year and a half ago).

Rafarrcc, they will want to get her into remission. So her light chains will need to be in the normal range. Her light chains and ratio are high. They probably need to be in the normal range before they consider another transplant. Light chains can damage the kidneys
Why not get on the transplant list and see if her kidneys will improve on the Darzalex?

According with the lab, these values are normal for a person who is in dialysis 3 times per week. But I am not sure if theses values will not damage the kidney in the future. Thank you :)
Thank you Donna for your answer. I saw in your bio that you have had a stem cell trasplant. I am really happy to heard that it works. My mum is afraid because she had a kidney trasplant before her diagnosis with MM. Due to prednisone her antobodies were very low, so a A Flue carry with her to hospital and after 4 weeks in ICU she lost the kidney again. The situation now is different because she knows MM. The hard question is, with this low levels in Kappa and Lamda chains, would the kidney work? Anyone here in a similar situation?
