Maria and @A MyMyelomaTeam Member,
The Multiple Myeloma Research Foundation has an article about Induction therapy. The typical 3-drug therapy is Revlimid or Pomalyst and Velcade, or Kyprolis, or Ninlaro. Then the last is Dexamethasone or prednisone. The 4- drug combo adds Darzalex or Sarclisa. They have a Patient Handbook (download or print) that is really helpful in understanding MM, and hopefully will give you some questions to ask your doctor. They also have a Patient Navigator that you can talk to. HTTPS://www.themmrf.org

What a great question to be asking as you prepare to start treatment! Yes, sudden rises in serum free light chain (SFLC) readings are something many MyMyelomaTeam members have noticed and talked about.

A quick spike can happen for a few reasons, including:

- Active myeloma progression — the abnormal plasma cells are producing more light chains
- Timing before treatment — readings can sometimes rise just before or at the start of treatment
- Infection or stress on the body — these can temporarily affect levels The SFLC test is actually a really important tool your medical team will use to monitor how well treatment is working over time. A high SFLC score can indicate more aggressive disease, which is exactly why catching it and starting treatment is the right move.

The most important thing is to share any concerns about your readings directly with your specialist before treatment begins. They can explain what your specific numbers mean for your situation and what to expect as treatment progresses. You've got this! 💪

Remember that patients express their MM symptoms differently via blood testing results. For some free light chains and ratio's the the best indicator. For me it is not. Mine is via m-spike/monoclonal results from a SPEP test. Plus mine shows up as bogus/cloned IgA immunoglobulins.
Talk to you doctor to get a better understanding of your type of MM. And it may take a few months for these blood test results to show themselves.
Lastly, I always look at my numbers a relative, not absolute. I see people post their numbers here and they are all over the spectrum and sometimes very different than mine. I think sometimes we are comparing tests that are using different measurement units.
Good luck with your induction. Get your test results and discuss them in detail with your doctor. For me just once cycle showed I was having a very good response.

Maria, miracles do happen. There are many on this site that inspire me. If you can see it, you can be it. I asked God to remove this from my body. In 5 months I was in remission. In the next 4 months I had completed SCT. I am 100 percent undetectable. I want to serve the purpose that God needs me too. Helping others be strong and know they can beat this. The power of positivity is mighty. Stay strong. Lynn

I also just started treatment because after watching my smoldering MM for 4 years, my lamba light chains hit the 14O0 hundreds and kidneys didn't look good. I don't know what is referred to the SFLC Shore. I'm new to everything and my oncologist office has not provided any documentation on even the names of the 5 induction drugs I was given two days ago. I hope yours is more forthcoming. I hope you do well in treatment.
