The timing of when to start treatment after a myeloma diagnosis really depends on several personal factors, and it's one of the most important conversations to have with your oncologist early on.

Some key questions worth asking your doctor include:

- How quickly does a decision need to be made?
- What happens if treatment is delayed?
- Which treatment options are available right now? For most people, treatment begins relatively soon after diagnosis, especially if there are active symptoms. The standard approach typically follows three stages:

1. Induction chemotherapy – to bring down the number of myeloma cells quickly
2. Stem cell transplant – to deepen the response (if eligible)
3. Maintenance therapy – to keep any remaining myeloma cells in check

The goal of starting treatment promptly is to reach what's called a "deep remission," where very few myeloma cells remain detectable. That said, in some cases doctors may recommend a short delay β€” for example, stem cell transplants have sometimes been postponed based on individual circumstances. Any delay should always be discussed carefully with your care team to weigh the risks and benefits for your specific situation.
