I was referred to an Oncologist who first diagnosed me with MGUS. After doing lots of research, I found I knew more than she did. That was frightening. On this site, other patients told me my cancer doctor needed to be a MM Specialist Hematologist/Oncologist. What a difference! This cancer is too complicated. If you don’t have a MM Specialist in your area, your Oncologist will be happy to refer you to one and collaborate with them in your treatment. Some can be done on FaceTime if travel is impossible. It is common to get a second opinion before starting treatment. Kappa light chains clog the kidneys and cause kidney failure, so time is of the essence. Watch your eGFR and Creatinine levels.

Are elevated free kappa light chain numbers alone enough to diagnose multiple myeloma?

Your elevated free kappa light chain levels (112 mg/L vs. normal 19 mg/L) and abnormal ratio (6.5 vs. normal 1.65) do indicate something is going on with your plasma cells, but they're typically not enough by themselves for a multiple myeloma diagnosis.

Doctors usually need several pieces of information to confirm multiple myeloma:

* Serum-free light chain assay results (which you have)
* M protein measurements through blood and urine tests
* Bone marrow biopsy showing the percentage of plasma cells
* Imaging tests to check for bone damage
* Additional blood tests measuring albumin, beta-2 microglobulin, and LDH levels

Your elevated numbers could indicate multiple myeloma, but they might also suggest earlier conditions like MGUS (monoclonal gammopathy of undetermined significance) or smoldering multiple myeloma. The serum-free light chain assay is particularly useful for predicting prognosis and monitoring disease progression, but it works alongside other diagnostic tests.

Has your doctor ordered additional testing like a bone marrow biopsy or imaging studies? These would help clarify whether you have active myeloma or an earlier stage condition.

I agree with others you need more testing. You definitely need a bone marrow biopsy.and protein levels tested. From what you have posted I would not say you have MM or are even high risk. The test for high risk is 20-2-20 . The first number is your ratio and you are only at 7. The last number is the bone marrow biopsy. To be MM I believe that number needs to be over 60. To be smoldering your bone marrow biopsy has to be 10 or higher. The middle number of 2 I do find confusing because the measurement given in results is not what you companer to that 2 you need to divide it by a 100 or possibly 1000. Any way I think you can relax for now and just get these tests or if you have already have them then check it against this test.
