Are suppressed and trending lower immunoglobulins an indication of MGUS progressing to myeloma?

Suppressed or declining immunoglobulins aren't typically listed as primary markers for MGUS progression. The main risk factors that doctors monitor include:

- M protein levels higher than 15 grams per liter
- M protein type IgA, IgM, or IgD (rather than IgG)
- Abnormal free light chain ratio in your blood

Your risk level depends on how many of these factors are present. Having zero factors means low risk, while having all three indicates high risk.

MGUS has about a 1 percent chance each year of progressing to multiple myeloma. Regular monitoring through blood and urine tests every 6-12 months helps detect any concerning changes early.

If you're noticing changes in your immunoglobulin levels, it's important to discuss this with your hematologist. They can evaluate whether these changes, combined with other factors, suggest progression and determine if more frequent monitoring or intervention is needed.
