If you're taking DarzFasPro or other mAb Maintenance Meds then a 0.1 MSpike must get the Lab test #123218 to see if it's False - if it's False, and therefore, Not Found, a blood sample clonoSEQ test would be perfectly appropriate to do.

Though if you have a much higher MSpike of 0.5 or more and elevated IgG or IgA levels, and elevated Infection markers of CRP, LDH, B2M, and high Kappa/Lambda and Ratio, then it's unlikely that you're MRD Negative.

Rather than just guessing - have them validate their comment based on actual Lab values and the removal of a False MSpike if certain Meds are being used.

Cat, my specialist got my first bone marrow biopsy tube from another facility and sent it for mrd testing. I had over 95% myeloma involvement in my marrow. I didn't see the point of doing the mrd testing. But, now I know he did it because they look at the different clone sequences to see if they have changed. I get mrd testing on my marrow every year. I'm not mrd negative, but my numbers are going down. I still have the same nasty clones I started with. Anyway, I don't know why he hasn't done yours.

If you have a 0 M spike and your numbers are good, I have no idea how he’d be able to tell

How would your doctor know if you're MRD-negative without testing?

Your doctor is likely making an educated assessment based on several factors they can observe without MRD testing. These might include:

* Your current disease markers (like M-protein levels or free light chains in blood tests)
* How your bone marrow biopsy results look under standard examination
* Your response to treatment so far
* Imaging results showing remaining disease activity

However, here's the important part: MRD testing is specifically designed to detect very small numbers of myeloma cells that regular tests can't find. Standard tests might show you still have detectable disease, which would make your doctor confident you wouldn't be MRD-negative (which requires less than 1 myeloma cell per 1 million bone marrow cells).

MRD testing uses extra-sensitive techniques like flow cytometry or next-generation sequencing that can detect cancer cells regular bone marrow exams might miss. If your standard tests already show measurable disease, MRD testing wouldn't change your treatment plan right now.

That said, if you're curious about the specific reasons behind your doctor's assessment, it's completely reasonable to ask them to explain which test results or factors they're considering. Understanding the "why" can help you feel more confident in your care plan.

I have to request it from my oncologist then I go to hospital to get kit and bring to lab and they draw blood for it and send it somewhere to be done then wait to see if it comes up on My Chart.
