My bone marrow was 10 to 15%. My FISH test was normal. FLC ratio was 35.2 and Pet normal. I don't have an M-SPIKE Lambda only. I don't know what to expect at my November appointment with my oncologist. I guess I am considered low to intermediate risk, but who knows. I wish you luck at your appointment. I was diagnosed 9/29/25.I dont have an M spike-Kappa light chain only. Im intermediate to high risk SMM because The marrow biopsy had 20% plasma cells and the Kappa : lambda was pretty significant at 39:1.. My FISH showed a 11:14 trans location and a 13q deletion. Im not sure how to interpret them (I think they are considered standard risk-not high) PET and MRI were normal. Im meeting with my hematologist this week to review everything. . I think Im also going to be in the observation and monitor protocol. There is definite anxiety (esp for the potential need for auto marrow transplant) even though all the advances have made it more of a chronic disease

Hi Gary, thank you for replying. How long have you had LC-SMM and was your bone marrow 20% at you diagnosis? And hwovwas your FISH and other test at diagnosis? This is so scary. I worry about it progression. With this disease, you don't know what to expect.

I also was just diagnosed with LC-SMM I have 20% plasma cells in the marrow and my FLC ratio of kappa:lambda was 39:1 I had transient mild elevated calcium ,creatinine and mild anemia which did not meet criteria to define active MM or high risk SMM and have reverted to normal spontaneously
So Im probably going to be on close observation but am a bit anxious that I will need treatment sooner rather than later
