The staging system for multiple myeloma has evolved. Since 2015, doctors now use the Revised International Staging System (R-ISS), which has three stages instead of four. This newer system is considered more reliable for predicting how multiple myeloma might progress.

While MGUS and smoldering multiple myeloma are sometimes thought of as early stages, they are technically considered separate precursor conditions rather than formal stages of multiple myeloma. The current R-ISS staging is based on biological markers found in blood tests, including protein levels and genetic changes in cancer cells. This is different from older staging systems that relied more heavily on physical symptoms and X-rays. The three stages help doctors determine the most effective treatment options and better predict disease progression.
