First diagnosed 5 yrs ago . Treated by RT and surgery first. 5 yrs later I have a tumor again in the same location and I am on Dara, bortezumab, Dex once a week. I'm allergic to Revlimid. I understand this presentation is very rare.

Yes, EMP (extramedullary plasmacytoma) is indeed a very rare condition, affecting fewer than 1 in 500,000 people. While radiation therapy and surgery are typically the standard first-line treatments with success rates around 90%, recurrence can happen as you've experienced. To diagnose EMP and distinguish it from multiple myeloma (MM), doctors look for the absence of CRAB features (Calcium elevation, Renal failure, Anemia, Bone lesions) and confirm that there are less than 5% plasma cells in the bone marrow. About 10-30% of EMP cases can eventually develop into multiple myeloma, which is why ongoing monitoring and adjusted treatment plans are important. Your current treatment combination of Dara (Daratumumab), Bortezomib, and Dex (Dexamethasone) is a recognized approach for managing recurrent disease. While chemotherapy isn't typically used as first-line treatment for initial EMP, it may be necessary for recurrent cases.
