How Long Should The Induction Phase Last, Which Is The First Phase To Get The Numbers Down Before Doing The Stem Cell Transplant?
I agree with the timeframes Donna and Mark provided. For me it was about 4 months. But there was a lot of prep work and time before my SCT. Many people in this forum get their stem cells harvested and stored ahead of time. The other thing you could ask your doctor is what can be done now to prepare for the SCT. Is he "eligible"? For me, that meant 3 days of extensive testing to see if I could physically and mentally/emotionally survive a SCT. Maybe those tests can be done now.
Also, I hear more and more people on this forum wait on the SCT. But they are getting good results from induction therapy. Like Mark, I had great results from my SCT, but it is a huge undertaking.
I also hope the addition of Darzalex helps. More and more this is being added to RVD induction therapy. I am on it for maintenance and have zero reactions and side effects. Many times it takes a lot of changes and additions/subtractions in therapy meds to get better/faster results. Just keep working with your doctor and push on them. And like Donna said, slow response is better than no response. Boy have I learned about patients with this disease.

For me it was a 16 week regimen of Dara, Velcade, Dex and Revlimid. Then after stem cell collection a bone marrow transplant within a month. After 100 days of “recovery” back on maintenance of Revlimid (lower dose) Dara and Dex. No Velcade. All went well and today, September 17, 2023 I haven’t felt this good in years. This was a reboot for the system!
I was diagnosed in Aug 2019 and started induction in Sep 2019 with the standard of care RVd. After plateauing for three months, we added Daratumumab and stopped Velcade due to increasing neuropathy. The DRd continued dropping M-spike and I had an ASCT on 7 Dec 2020. Went on Revlimid maintenance in March 2021 which is continuing now. The past five months we've been monitoring a slow rise in M-protein which has plateaued for now. Will resume Dara-Pomalyst-dex and maybe a 4th new drug when we assess relapsed/refractory state. No SLiM-CRAB labs or symptoms yet, so still maintaining this watch and wait approach.
Marlea, I was referring to I guess 'normal' induction is 3 to 6 cycles. It took a year for me to get to transplant. Lots of complications. Slow response is better than no response or going backwards. Peope on here get to MRD negativity after 2 months of induction. Not me. I'm still MRD positive after 6 and a half years of treatment. But my meds are still working. They might have to tweak your hubby's meds a little. 🙏💜
I think usually 12 to 16 weeks. Then they reassess to see if we are having a good response💜
