It's hard to find what a bad number is on the internet. If my IGA is 500 and it says High, is 10,000 really where Myeloma comes in? What about the other numbers? Especially the percentages on the FISH report.

If I understand your question are you asking how far above normal matters - correct?

That’s a tough one that probably only the oncologist can answer.

When my IgA was 5915 it was obvious. When it returned to 570 they were very pleased but said I still had a ways to go (440 was normal high - I think). Also different Lab have different ranges.

Best is to look at a collection of items. I remember my Fish stated percentage of bad plasma and type of elements. Past seeing 80% bad, everything else about it lost importance.

When I got monthly Labs I learnt to look at my IgA and my MSpike as my critical values. IgA and other Ig’s became lower than normal indicating a weakened immune system due to the Meds and MSpike varied around 0.1 or less. I found on this forum that when taking Darzalex FP the Labs need to be tested in a special way to avoid an error in the MSpike results (up to 0.2 too high). I mentioned this to my Doc. Their testing Lab was not accounting for this. They are now.

Only thing to do is ask about everything that’s high (I did) and have them tell you which ones matter

Most of the abstracts Ive read put MGUS at less than 3% of serum monoclonal proteins, less than 10% of clonal plasma cells in bone marrow and no CRAB symptoms, SM is less than or equal to 3% serum proteins, less than or equal to 10% clonal plasma cells in bone marrow and no CRAB symptoms and MM is greater than 3%, greater than 10% and CRAB symptoms. Ive read that SM is basically MM with no active symptoms so most Drs wait til symptoms start to treat. I had 12% serum proteins, 60% clonal plasma cells in bone marrow, and all the CRAB criteria. My IgG was almost 12000. Not sure if thats what u were looking for. Hope it helps a little.

I have kappa light chain MM but don't usually get an M spike no abnormalities or deletion for my tale tell is my kappa/lamda ratio. At diagnosis stage 3 numbers where in the 4300 range.

Thank you responding. I have good and bad numbers. My IgA is 591. It went up 72 points in 6 months. I don't like to see my gene rearrangment in 53% of the nuclei and then be told it's okay. I'm going to check out another specialist.
