Have Kappa light chain MM. Did a 15 month treatment of Dex/rev/darzalex. Started with Kappa labs at 1200 and KL ratio of 250. Now at 14 kappa and K/L ratio at 5.27. Doing monthly treatment for Daratumaub slow push injection and Igif IV. Paraproteins started at .5 and have stabalized at .02 for last 6-8 months. What does this means in terms of how long I can expect to stay in maintenance without MM returning?

How long can you expect to stay in maintenance with stable paraprotein levels at 0.02?

Your stable paraprotein levels at 0.02 for 6-8 months is encouraging and suggests your maintenance therapy is working well. However, there's no definite timeline for how long myeloma will stay controlled.

The timing of relapse varies significantly from person to person. Research shows that about 50 percent of people relapse within roughly 27 months after completing first-line therapy, though some stay in remission much longer.

Several factors influence how long you might stay in remission:

* The longer you remain stable on maintenance, the better your outlook tends to be
* People who relapse more than a year after starting treatment generally have better outcomes than those who relapse earlier
* Your specific genetic characteristics and how well you respond to treatment play important roles

Your stable numbers over the past 6-8 months are a positive sign. Continuing your maintenance therapy as prescribed is vital - studies show that sticking to maintenance therapy helps prevent the disease from returning.

Your healthcare team will continue monitoring your paraprotein levels, kappa light chains, and other markers through regular blood work to catch any changes early. If levels remain stable, you could potentially stay in this controlled state for years, though maintenance therapy may continue indefinitely as long as it's working and side effects are manageable.
